2. Submit the following documents:
   • Child’s Birth Certificate- Akte Kelahiran (Photocopy)
   • Parents KITAS/Passport (for expatriates) – Photocopy
   • KTP (Photocopy)
   • Family Card (Kartu Keluarga) – Photocopy
3. Payment
